A historical and anthropological review about this  pathological entity was made from 121 texts collected in public and private libraries around the region.  Results. Many files about “empacho” exist in Central America since the sixteenth century to our days. The concept, the epidemiology and prophylaxis, etiology, signs and symptoms, even treatments are similar from Guatemala to Panama. Discussion and conclusions.  “Empacho” is widely recognized by the popular and native people in Central  America. At least within bibliographical references, it is more frequent in Guatemala, Nicaragua, and Costa Rica. Every country along the region has shown an important conflict between folk medicine and biomedicine, it has been underestimated by academic physicians that approach it as any other digestive disorder such as dyspepsia, irritable bowel syndrome, intestinal obstruction, or gastrointestinal infection. “Empacho” is still, a long-lived and day-to-day health problems. Introducción. El empacho (o “pegas” en Costa Rica) es una enfermedad de construcción popular que refiere a trastornos del aparato digestivo debidos al estancamiento o detención parcial del tránsito gastrointestinal. Métodos y Materiales. Se hace una revisión histórica y antropológica de esta patología a partir de 121 textos recolectados en las bibliotecas nacionales, universitarias y privadas ubicadas en la región. Resultados. Existen documentos sobre el empacho en Centroamérica desde el siglo XVI hasta la actualidad. La concepción del empacho, su epidemiología y profilaxis, la etiología, los signos y síntomas, así como los tratamientos, son similares desde Guatemala hasta Panamá. Discusión y conclusiones. El empacho es ampliamente reconocido por los sectores populares e indígenas de la región centroamericana. Su frecuencia por lo menos en términos de referencia documental se da más en Guatemala, Nicaragua y Costa Rica. En todos los países de la región existe un conflicto entre la medicina popular y la biomedicina, siendo subestimado por los médicos académicos que lo interpretan como un trastorno digestivo equiparable a la dispepsia, al colon irritable, una obstrucción o una infección gastrointestinal.
